In-universe? Not directly, but a lot of the innuendo doesn't make sense if everyone knows she's trans. Granted, the issue here is metatextual regarding how Willis handled Carla since the start of the strip.

It's an issue inherent to depicting a nonbinary person in a visual medium. What makes a person nonbinary is how one relates to the gender spectrum. However, one can't really depict such an internalized decoupling visually, at least not without significant stylization of presentation, which in turn means that visual depictions of nonbinary people usually default to "is X but does Y thing."
It’s the easiest way to communicate that he is mentally ill.
