I'd guess that the combination of being colourful, the model work, the narration style and general simplicity of the stories (they were after all based on stories told by the Rev. Awdry to his son who was ill with measles) all adds up to be an incredibly enjoyable to people on the spectrum.
They didn't want to have to put subtitles on it.

I think it's a combination of these
for whatever reason Trains are often popular with people with Autism(I'm sure someone has done a study on this or something), so taking that and adding a touch of humanity to the mix probably just increases the appeal
also it probably helps that the older stuff in The Railways Series(both books and the earlier shows and toys) were rather high quality products for their intended age group

They had to, he did the first two seasons while he was a barely functional alcoholic then sobered up and decided to ditch the show and go on tour with his All-Starr band. They replaced him with another Brit in the UK and we got Carlin in the US.
It never occurred to me that they would be small.
I've always thought of them as small. There's a few scenes in the later CGI series where they experimenting more with camera angles like showing things from the Fat Controller's viewpoint and it's uncanny how massive they look.
It really depends on what scale they're shooting. The normal size is 1/32 scale, then there some at 1/16 scale to be used when working with engines that are supposed to be smaller, and then some larger models that are used for close ups with the larger human figures.
