We know that they still have their fully developed teenager minds when they become babies again, as that is how it worked with Marco. They just can't speak except in baby talk, and they are helpless in their baby bodies, getting breastfed and then bottlefed and finally spoonfed baby food, being burped, being trapped in a crib, having their diapies changed after peeing themselves, etc. They probably try desperately to communicate and it is interpreted as them acting up and misbehaving, leading to spankings. Mariposa probably finds it humiliating being changed by Marco now, she wants him to recognize her as a teenage girl and respect her privacy but he just sees her as a baby.
