If you like the game then sure there's no harm in going for all achievements. Some just go for achievements just because of achievements.
If you fail the achievement and can't go back to do it, do you go on with the playthrough or reset?
This excludes one time death type of achievements of course. I usually just go on with it even if it's a lost cause.

I usually do one playthrough normally, then I look at the achievements list to see if it's gonna be a massive pain to get the rest. If it only takes another playthrough to mop them all up, I'll do it, otherwise I forget about it, and fuck any games that has timed or online achievements.
I like it when games just have difficult achievements, like DX:MD having you play the game on ironman on the hardest difficulty, or complete it without killing anyone or setting off any alarms.
