He's unironically a severe alcoholic. He should have never been drinking, given his lifestyle and past in mind.

Most of his health issues line up with associated issues related to alcoholism, most noticeably his nerve pain. And surprise-surprise this includes low Vitamin b1 which is essential to healthy mood capability, not to mention the excess then fleeting dopamine and low GABA.

As bad as things sound, the sooner he quits the better. Most of his numerous health issues will vanish and as long as he ceases alcohol and even caffeine he should be functional with enough time. And with more progress he could make a comeback. Not exaggerating.
