They locked the door when they went out, they just left it unlocked while they were in there.
They live in a nice apartment building in Manhattan so it's unlikely people could just come off the street and go in.
We know they lock it when they go out because you often see them unlocking it when returning home. One time Monica comes home and Ross, Joey and Phoebe are in her living room, to which she says "You know I gave you those keys for emergencies", implying they unlocked the door.

There's a whole episode in season 1 when they rush out to see some parade balloon and they lock themselves out.

Not true, for example in this clip their door is unlocked in the middle of the night (and so is Joey's and Chandler's door).

You don't need to unlock those doors from the inside. And Joey and Chandler's door obviously is locked otherwise Rachel would just open it rather than banging on it.

1. When they retake a scene, do the audience have to pretend to laugh at the jokes the second or third time round?

2. What happened when Jennifer Aniston was naked in that scene, did the audience all see her exposed vagina?

3. Does the story make no sense to them if they presumably film all the coffee shop scenes in a row?

I never considered this. I can only assume it was filmed without the audience and she wears those skin coloured panties they use for films - they have a name I just can't remember.

The scenes that aren't done live are filmed beforehand and are shown on tv screens. The stories have to make sense to the audience so they can get good laughs.
