Why would a growing city put up a wall directly around the existing buildings? Its growing, they need room in the walls to keep building.

And then people start building next to the walls provided no enemies destroy them, and then 100 years later new walls are built and the original walled area becomes the "old city" or rich people area. Rinse and repeat

That's only the central city. The entire land within the outer wall is about the size of France. There's plenty of farmland out there. The central city slickers import food from the farmland. I don't know why this is hard to understand.

A longer wall is harder to defend.
You don't just build a wall for fun around some unused field.
It is even perfectly fine to have houses for the plebs on the outside.

When someone attacks the city, the plebs will be let in and the houses outside will be burned down before the enemy army reaches the city... which is realistic, because those armies are slow and you have lots of time to prepare for a siege.
Inside the walls you have the rich people, some big buildings that can be used to temporarily house the plebs and food storage.

Pic related is the wall from vienna, at the time it got built. Everything thats inside is alraedy filled up. This wall did successfully withstand a long-going turkish siege.
