how much experience do you have ? im afraid my older brother is going to commit seppku if we dont treat his acne scarring soon. what do you recommend

You can do so much with an accounting degree and just a couple years experience but the type of people who become accountants are too cucky to leave their current cubicle and take a risk

I'm just a resident. It depends on the scar type. Not all acne scars are the same. I'd have to see a photo to judge. There are tons of treatments available though. What has he tried? I'm sure there is something else to try. There is always surgery as a last resort.

Accountant here. What do you recommend for my psoriasis?

Hydrocortisone cream and Dovonex work a little with a lot of treatments. I'm on two drugs to weaken my immune system for two other autoimmune disorders. Only thing that ever worked really well for my psoriasis is prednisone.

hes undergone “ microneedling” which didnt help at all. we also took him for a laser treatment last year which seemed to help quite a bit . after the post-op peeling faded he became homebound and depressed again because his scarring is still noticeable according to him.. he acknowledges the improvement but we’ve had to hold off on more treatments due to financial reasons . they said he has “ ice pick” and one other kind of scar

Have you tried phototherapy? Narrowband UVB can be pretty effective, but it's hard to find in some parts of the country.

Yes full body many years ago. It works but with a lot of treatments. Leaves like a white dry powder stuff. Probably gives cancer. Not anything better than hydrocortisone.

I usually sleep face up and I can tell I need a new pillow case when my neck gets more pimples.

The biologics are honestly the best treatments in my opinion (e.g. Stelara, Cosentyx, etc.), But it sounds like you are already immunosuppressed with your other meds. You could try topical tacrolimus or pimecrolimus. Tar is a classical treatment but messy.

I used to use coal tar shampoo. Horrible stuff.

I'm on methotrexate for arthritis and meracaptopurine for ulcerative colitis. These don't help my psoriasis. Neither did humira when I was on that.

No problem.

Stelara and Cosentyx are supposed to be good for plaque psoriasis as well as psoriatic arthritis. You might be able to drop the methotrexate if you go on one of those, but I would check with whoever is managing your mercaptopurine if it wouldn't be contraindicated. In med school, they really emphasized that methotrexate has terrible side effects. It might be nice to swap it out if possible. You getting regular colonoscopies?

The methotrexate is really low dose. Every five years for colonoscopy. Cost me almost $1800 last time with insurance.

Oof. That rough, man. It seems something insurance should pay for. Medical insurance is really out of whack in the US. $1800 every 5 years is better than than getting colon cancer though. Also, at least you don't have Crohn's disease. Every surgeon I've ever asked has agreed that UC is the "better" version or IBD to have.

I worked on a divorce case once that was some hillbilly who was ungodly rich off of generational land ownership and oil being discovered on the land.

I couldn't believe how much money the guy had. He was from bumfuck nowhere, dumb as hell and the divorce was getting nasty. Just goes to show you do not have to be smart to be rich. Nothing against him either, he struck the lottery by being born.
