what did to heart do that wasn't already a part of the industry beforehand?
It made visual novel adaptions popular and mainstream, laying the foundation for the 2000s visual novel adaption boom. By virtue of being a romance visual novel, of course it had a self-insert protagonist (which it didn't do first, but it popularized), and because it was a trend-setter, the industry got flooded with self-insert protagonists.
Not to mention it essentially established the "romance harem high school SoL" genre as a staple of the industry rather than something that just happens in OVAs.
Plus it was a mega franchise that milked its otaku fans for hundreds of dollars with how much multi-media products it had.
