I think the simplest solution is the most logical one. Tooru killed the Head Doctor years before the events of the story, and his Stand has some kind of shapeshifting capabilities. That's why his Stand looks like the Head Doctor.

Shapeshifting and illusions can be thrown into the same bucket, and we already had Whitesnake, which could both create illusions and steal Stands/memories. Do I think there's only one stand in play? No. But it's not impossible

Whitesnake at least could be justified through the stand discs, it's primary ability. It would be weird if Tooru's stand is both the calamities AND the illusions when so far there doesn't seem to be any connection between them
