That's an actual plot hole. I can only assume that Jotaro's already high willpower and use of SP allowed him to continue using it without DIO's presence. Holly doesn't use her stand, so it may just be dormant now.

This is stupid. Koichi would have died, but survived because of Crazy Diamond. If you recall, Koichi had to develop his stand through character development, and it was just a useless egg at first. Hence, he had the potential to be a Stand User later on (as anyone is capable of it), but had it forcibly awoken in him before he was ready, and only survived due to outside influence.
