All have a generic fantasy world setting, a generic protagonist and some harem of girls that keeps expanding with every season because character development is hard so the writers would rather shit out new characters endlessly.
Where is my underwater isekai, science fiction space travel isekai, steampunk isekai, and so on.

The biggest and most important difference is usually the ability of the author. Even if getting less generic settings and events more often would be nice, that's usually not the biggest flaw.

Except there's nothing about isekai that makes it have to usually be in the standard fantasy world. I like many of them, but there's no reason there can't be more in other settings.

We know that different people will think up of different stories and different characters will make the story flow in different ways. But you cannot help but see similarities, so what matters is execution. Re:Zero executes the whole fantasy game setting by putting a setting of realism in a world of game-like fantasy against a realistic protagonist with a game like ability. The whole "game" aspect is pretty much secondary and not in your face (looking at you status screens and skills), plus the way Subaru has to struggle to achieve his goals is a stark contrast to the escapist nature of these stories. Look at Mondaiji too, it pretty much has most of the elements of an isekai story but its executed so damn well that I actually forget from time to time that it is an isekai story.
