It always seemed like an especially contrived coincidence that he lost control right as he made a bullshit suggestion to commit genocide to his second favorite sister.

It was definitely foreshadowed that he would lose control of his power.
That he'd say one of the few things that could ruin his plan at this very moment is awfully convenient. And that all of this leads to half of S2 being a lame retread of S1 is just annoying, lazy and ruins the pacing.

It was heavily implied. C.C. said something akin to "Geass reveals your innermost desires" in R2. The implication being that Lelouch's Geass misifiring revealed his true desire to destroy Britannia even at the cost of Euphie.

When he hypnotized Euphemia, he tried to disable the command as soon as possible, but he couldn't. It was only later on that he realized that this (convenient) twist could actually be useful for him, but even so, C.C had to comfort him after killing Euphemia.
