But his reaction was ridiculous. He thought he was fake because a serial killer (who he knew was trying to mess with him) made a ridiculous assumption and because his brother had been wanting to ask him something but hadn’t yet. I understand being paranoid but fuck me, gather some more evidence and insight before throwing a fit.

I'm not a big fan of this part, but in the manga/Brotherhood it ends quickly with a really satisfying confrontation and heart to heart between the two brothers, so it was worth it.
Fuck 2003 though, it drags it out for no reason with Al running away from the hospital and getting involved in some stupid shit, all for an underwhelming resolution. Fuck it.

He made a decent point despite knowing he was saying it to get into his head, which it did since he planted the seed of doubt. The killer guy didnt care either way which was a good contrast to personalities and mentalities about such matters
Al's reaction had more to do with his stress piling up since his soul transfer than any logic or believability to the theory that he wasn't a real person. He doesn't eat or drink, he doesn't need to breathe, he's lost most of his senses other than sight and hearing, and the only thing keeping him "alive" is a fragile old blood drawing. He went from being some short young teen to having a like 7 foot tall metal body overnight and everytime people realize the armor is empty they call him a monster. After a couple years of enduring this and then recently his brother acting distant and obviously hiding something from him, the comment asking if "Alphonse Elric" was even a real HUMAN was enough to make him snap.
Basically, the drama was that Al needed a reminder that he was still human and not just a living set of armor.
OP, you're forgetting that Alphonse literally has no physical body and is a suit of armor, and Edward burnt their childhood home down, so there's no physical evidence left that his memories are really true.
Al was using his identity and memories to anchor him to the idea that he was still human despite being a soul struck in a set of armor. Then he sees a bunch of fucked up shit across his travels with his brother and knows its possible to do a lot of shit with souls and the like through alchemy. So when someone questions whether his identity as Al is even real it digs into a big sore spot as its essentially implying that he's not even human which he finds it difficult to disagree with given his current body.
