for a simple reason is not attractive simple as that besides is mostly cowboys while the other theme is really fantastic and perfect for isekai.
for example the game with an old west setting tengai makyou 3 barelly worked and several got offended and triggered by the game

Stop fucking asking this question you retards, isekai grew out of medieval fantasy stuff, that's why it's all medieval fantasy stuff, it's functionally a subset of that genre. It's married to medieval European fantasy like noir is married to modern urban environments.

All jokes aside, isekai is easier with the standard fantasy tropes because it's [the standard fantasy tropes].
Right now, the anime industry is co-opted by the lowest common denominator: the consumer. The consumer doesn't want variety, the consumer doesn't want new, the consumer doesn't want innovation, the consumer doesn't want fresh ideas.
The consumer wants waifus. they want moe. They want safety and comfort.
You wanted this, you voted for this, you decided this, so this is what you get. Enjoy your seasonal mediocrity.
That’s a good point. I tend to think of it as a genre on its own but it really is just a sub-genre of fantasy.
So to go along with this question, are there any really good anime with an Old Western setting or vibe?
The wild was "wild" because it was on the fringes of society. The only thing maintaining law and order was 1 sheriff, sometime they were the bad guy because they just like conducting everyone's lives and wearing titles like a hat.
And maybe the reason the town is a ghost town.
The medieval ages had guilds. They were a social institution. Like proto-colleges, but with more authority and influence.
The medieval setting is better suited for an isekai narrative. Because you have characters that want to do things and be productive.
A west is where people went to live on their own. To get away from cities. They want to live by their own rules.
In that way, it wouldn't mesh with RPG mechanics either.
The Japanese like stories about making the best with what you got. And following rules. That's why you get isekai like Shield Hero who picks the weakest and unwanted Slave, and made something of her. And the countless other manga that are like "I became the most powerful with this underrated skill"

The stories you think are true are almost entirely made up and fictional, it's literally the equivalent of thinking Europe has real dragons and goblins and shit.

And btw America had better gun control at the time than they do in modern day, mostly because of how shit the guns were and ammo wasn't exactly something you wanted to just waste.
The actual reason is because a bunch of RPGs use a generic medieval fantasy setting and having actual stats appear would feel insanely out of place in any other type of setting.
Medieval European settings are a dime a dozen.

Anyway, yeah a lot of myths and legends came about during it like Billy the kid.
That's what caused the wild west genre to be made, but a lot of it is fictional.
Medieval Europe didn't have dragons, feudal Japan didn't have kitsune or magical tanuki, and cowboys were boring most of the time.
Old isekai was propaganda to inspire men to hop onto trains and ship out to fight wars in other countries. Romanticize the idea of being out of your depth.

"cowboy" is a complicated term that a lot of people attach a lot of different meanings to, like "gunfighter" and the like. Most are trumped-up bullshit, like you said.
Still, it was a magical, mythical time that *could* be explored in anime.
The actual problem is that there are genuinely *hundreds* of books, movies, and television series from the 50's all the way up to modern times that thoroughly and completely explore the "Wild West"...
But since they're not japanese cartoons with uguu moe moe catgirls eating cake and fighting over the strawberry on top it falls outside of Zig Forums's sphere of relevance.

I’m pretty sure the first Isekai was The Wizard of Oz, which 100% isn’t medieval fantasy. And if it’s not Oz, then it’s Alice in Wonderland.

Digimon was an isekai and was 100% not medieval fantasy, and I’m pretty sure that predates SAO which is the influencer for all modern isekai.

I'm more suprised there weren't any cowboy mangas that where popular. I know in the 60s 70s and 80s pre economic crisis american Westerns where incredibly popular in Japan. Like even to this day things like Country western music and stuff is oddly popular over there
