I only know about classic alt-manga from GARO.
Is there anything similar but more recent?
I pretty much follow Comic beam on a monthly basis. Best alternative manga magazine in my opinion However I'm not sure how much is Translated.
This. I have read a little Garo stuff, and some stuff that gets described as "avant-garde" manga, but other than Garo, I don't even know where to start with finding this stuff.

Maybe run through it slow with a translator application open next to you?
I am still learning, so I keep tabs open to look up kanji and unfamiliar phrases as I go. It is tedious, but worth it to be able to read whatever I like.
Drawn & Quarterly has translated quite a bit of Garo artists.
I'm not sure if there are any completed alt-manga scanlations, but a group picked up Nekojiru some time ago and is releasing new chapters every 2 to 4 weeks.
I find them interesting but I'm not that in touch with the basics of Japanese culture always let alone the alternative strains of it. I feel a lack of connection to the content a lot of times.
But still, there are some gekiga and the like that I find very fun.
The Pushman compilation has something for everyone so thats a good place for people to start.

Yoshiharu Tsuge is one of my favorite mangaka. I like Suehiro Mauro, as well, though mostly for the art.

I like his storytelling too, but it never leaves that much of an impression compared to the art for me either to be fair. The visuals are definitely supposed to be the star of the show
Picked up the last gasp english version of Panorama Island recently and I really like it, it's a nice hardcover too.
Something like Panorama Island is an adaptation, so his storytelling works to enrich the existing story. His regular stuff is almost purely driven by the art.

something close to te heta-uma style of the garo cover you posted is Tokyo Zombie by Yusaku Hanakuma. This is my print (Italian) but I think you can find an English translation

sometimes I'll just type in a search word like "water" and there'll be some unsettling story about a girl turning into a fish or a fantastical sol with whales flying through the sky, whatever.

Lovely strange dark is hands down the best group currently scanlating. I like anata no motokare too however unfortunately their main girl/guy is having a really bad carpal tunnel thus everything is very slow.
