Add some more 'horror' style at parts.
Grey Fox slashing people in hallways, The Sorrow's bossfight, Psycho Mantis' bossfight, these moments are ripe for a solid bit of psychological fuckery
At least try to call back to Shinkawa's art style
Don't be afraid to go over-the-top at times, but don't go full Twin Snakes (though I do like Twin Snakes)
Give V a better conclusion. I like the bathroom scene but a more definitive conclusion that wraps up some loose ends would be nice. Plus the whole "YOU are Big Boss" thing won't work as well in an anime format.
