The manga skips a couple of arcs but over all it does well adapting for the most part. However its just better to read the novels since the manga does its own thing sometimes.
He probably assumes he’ll just find out later. It’s possible he’s curious and forgot to ask, or feels like it’ll start a fight if he does. It’s also possible Touma hasn’t really noticed because he’s always distracted when he’s with Accelerator. I don’t think “he doesn’t care” or “he thinks he did it” is right

Was the staff the producers chose. It kind of worked for first half of OT, but for the 3rd season, their argument that was 8 year old content wouldn't sell so they'd not like to invest so much into it and just continue on. It backfired and for the first time they apologized and aimed to change that with subsequent projects, which Railgun T got. Railgun T's happenings was largely born from learning from Index III and prior.
Not really. Otherwise the movie wouldn't have been like such quality.
