The last chapter is either a dream or the afterlife, the final page is the two of them holding hands but Gennosuke has his missing arm so it can't be real. It's generally accepted that Mie committed suicided because while she hated Seigen she also hated the samurai caste system that Gennosuke blindly followed.

It's weird, I honestly thought she would have gone with Seigen after he killed her father given how she hated what her father forced on her, plus women naturally became his groupies. I think her heart was wavering up until the very end though.

mie committed suicide not because of gennosuke but because she couldn't bear to see seigen die, she was still in love with him, mie in the novel I never love gennosuke, she only used him for a revenge tantrum that even she didn't believe. Gennosuke did not do anything wrong to Seigen's corpse after the duel, the message is totally different. Itsbullshit and yamaguchi knows it

If Seigen offered Mie the simple deal that he kills her father and they take over the school she probably would have taken it. She'd have accepted it from anyone (who is capable of it). However Seigen was blinded and then started scheming to destroy the entire swordmandship school in retaliation, including mudering a child pupil. I don't think Mie was down with that. She didn't want Gennosuke to die, he defied her father to give her the seashell so she thought he could be redeemed from his blind obedience, but Seigen wanted to kill him so that was a losing scenario for her.

Looks to me the manga improved the story by a long shot.
