Hashirama most likely died as result of overusing his healing abilities. That's probably why Tsunade knows that using the byakugou grants a shorter lifespan. Kimimaro probably didn't thought about that bacteria and/or viruses can enter the body through openings, what's worse he sometimes returned bones to his body even when they had blood on them. Getting a disease was expected to occur. With Itachi, I wouldn't be surprised if Danzo was the mastermind behind his condition, specially when people like torune's father were his henchmen. Mangetsu was a similar case as kimimaru. He might gotten contaminated by a liquid when he was on his water form.

I'm rewatching/reading naruto, and its really impressive how bad it is. The plot has tons of important holes and is insanely boring. Once obito comes into the picture the entire plot revolves around him until kaguya shows up - even then that's related to him.
I really feel like naruto was only successful because it has good fight scenes.
