Ultimately, the problem with kemonomimis is that they have a human skull structure, and the funny animal ears are just cute hair accessories. If you put the ears sideways, as they're meant to be, then the animal ears stop making the kemonomusume look cute, and it just becomes an ugly freak.
So, artists use the trick of simply hiding the place where the humans ears are by having hair conveniently be there.
Just like almost all anime has nude women with godiva hair that allows them to hide their nipples and even vagina by being conveniently placed around, defying even internal logic.

The thought of them having four earholes is kinda gross and disturbing, and above all it just look like shit and a lazy design. Two ears not only look better but also makes the character feel much more authentic. If they also have human ears then the animal ones just look like an accessory. It feels like a last minute addition by the artist rather than a proper character design.

Not always, it's rare nowadays but I remember in older stuff it was more common for the animal ears to come out were the human ones would be. But even ignoring that, the fact that they have visible human ears will always make the animal ears look even more like an accesory or an afterthought.
Personally I think is pretty good actually. It's kind of uncanny on that illustration because it's has no hair so the ears look kinda oversized but otherwise it's a good basis for a design.
It's always gonna be at least a bit weird if you think about it, like most fantasy designs, but four ears will always look worse than two.
