I've volunteered in state funded orphanages. Well, more like homes for "wards of the state", most kids weren't actually orphaned but had super shitty families. You'd be surprised at how much abuse goes on in them and how little the police or any authorities care. This was in one south american country and two different european countries

If abuse is so common and transparent that even volunteers can see it, I'm not surprised that killings could happen as well. Giving kids absurd amount of drugs (to "calm" them) is very common in these places and one specific place (in Chile) it was a confirmed cause of many deaths, and they were swept under the rug for many years. Forcing these children into prostitution is also quite common, and there isn't really that much distance from that to selling them to psychos with murder fetishes.
