Eh, the EoE scene with the spear in the EVA might be a sex reference (as are a lot of things in that movie) but it still leaves open to interpretation whether Shinji consciously perceived it as such.
So that still leaves the question, though, of why he didn't go for it anyway; or alternatively see sex as a way OUT of how depressed he feels.

He was completely messed up, distraught and anxious, and then a woman he’d begun seeing in a motherly/sisterly role made a move on him... I couldn’t blame him for pulling away, even when I was a kid I thought she was over the line there.
Blushing over embarrassment is different than blushing over being in love, see how miserable he looks in the images you posted? Now look how happy Shinji is when he blushes at Kaworu.

He craves love more than sex. Kaworu clearly put more effort into getting to know him as an individual than Misato "well he's a teenage boy so surely having sex with a woman will cheer him up" Katsuragi or Asuka "well he considered kissing me in my sleep so if he's not holding me when I'm my fully awake, mean-spirited, negging-him-through-taunting-about-his-dead-mother self clearly it means he's no longer attracted to me" Langley "wait what's all this sticky white stuff all over my hospital bedsheets?" Soryu.
EoE leaves open to interpretation whether Shinji consciously realized the parallels between "penetration" of the EVA and sex (he's a bit on the naive side, as evidenced by his nonchalant reactions to Kaji's "is she still wild in bed" remark). Maybe he's just smiling at seeing Kaworu because he misses the guy?

He was trying to run away again. Rei wasn't Rei anymore, Kaworu was dead, and Misato was telling him to get in the robot, so the only one left he could use as escapism was Asuka. Misato noticed that and immediately called him out. And when Maya was screaming and begging him to go and help Asuka because the Mass Produced Evas were literally killing her, he stood there and did nothing. At least manga Shinji had the balls to tell Yui to wake the fuck up and move.

I've never understood the "help me" thing, though. Did he not hear Misato's phone conversation with headquarters about Asuka fighting off the EVAs? Wouldn't that make him MORE motivated to get in the EVA, if only to be able to establish radio contact with Asuka, let alone use it to rescue her like he used to?

Again, I'm aware that even before the EVA was stuck in bakelite, he was reluctant to get in said EVA. But once he saw the EVA stuck in bakelite that was a whole different scenario.

He doesn't enjoy it, though. Shinji didn't like who Misato and Asuka were as people, he mocked Misato for being slobby and mocked Asuka for crying about her mother. His attraction to them was shallow and they knew it and called him out on it. He accepts Kaworu because he loved who Kaworu was as a person beyond the superficial "he's hot", Kaworu was everything that he wanted to be.

Early on, after she'd only been his commanding officer and roommate for a few weeks. I would guess that toward the end of the series, after realizing the shit she went through, let alone how much of that was for him, he wouldn't dare mock her over something like forgetting to make breakfast.

Not to her face... except in the immediate aftermath of her calling him out on trying to kiss her in her sleep, at which point he would've thrown damn near ANYTHING at the wall trying to fight back. Usually he's meeker and milder around her than damn near anyone else, which shows either how intimidated he is by her or how enamored he is to her. (Possibly both?)
Speaking of which did either of them call him out on "shallow" attraction to them? I know Asuka called him out on how he acted on it, but not necessarily on it being shallow.

He actually saw her crying on the table in one scene and ran away without saying a thing. >Not to her face... Maybe not, but he still mocked her for being a kid just because she was crying about her mom which is kind of fucked up. This isn't the fault of anyone else mind you, it's Shinji's. Throughout the entire series Shinji has a serious probably being unable to put himself in the shoes of anyone else and empathize with them. He's too convinced he's the only one that's suffering, and refuses to see anyone else outside of superficialities because they're all "others". The only ones that don't fit into that are Rei and Kaworu, Rei is more like a subconcious version of himself and Kaworu is his ideal self - if we want to take Anno's words and apply them to Shinji.

Again, in the manga Shinji was depressed but at least he still wanted to fight to help Asuka. In EoE, however, Shinji had given up since the very beginning.
