It's funny because he has literally no direct references to drugs in the story.

Giorno's froggos reflect damage. Giorno gives birth to froggos, Diavolo kills them.

When did Diavolo kill any of Giorno’s frogs, and how could he do that if they reflect damage? The only one who kills Giorno’s frogs is Giorno himself, as he kills them as soon as he’s done using them as slaves.

Good thing Giorno never left a hundred of those damage reflecting frogs wandering around a busy highway.

Diavolo - literal infinite , non-stop suffering. You could even speculate that he suffered similar fates to other villains within his own fate. Objectively the worst.

Kars - many years (potentially hundreds of thousands) of boredom, frustration, and helplessness, but it did eventually end when he stopped thinking.

DIO, Pucci, Valentine - they just died.

Kira - died, but his ghost basically got a happy ending for some reason.

To elaborate:
Diavolo never actually dies in requiem, he’s just always NEAR death. Eventually he’ll realize that he’ll never actually die from any of those illusions so it really isn’t that bad. Also I assume that his body in the real world will eventually die for real so the requiem illusions will stop.
Meanwhile Kars is stuck in space for an eternity, endlessly wandering through nothingness without even thinking, just feeling the void. It’s fucking scary.
