Naruto failing the exam three times comes from the same pool of ideas that Naruto literally being the Kyuubi came from. That being, his original concepts for Naruto as we know it. That is why this discrepancy exists.
If you want to retcon an answer though, easy answer would be he begged the Hokage to let him take the graduation exam, and he failed, and this was allowed 3 times. Or, they had 3 mock exams beforehand. Easy to fit an answer in here.

because most manga authors have a point A and point B planned, but anything beyond that is just a haze. If the manga gets picked up, all of a sudden they have a cash cow that they never wanna stop making, so now all of a sudden they need a point C and D, and lots of shit gets lost when they start to realize they can just write the same work forever.

It literally happens to every manga that is being written to sell a concept instead of being written as a passion. One Piece is a great example again, Oda likes to claim he has a plan, and that the ending he's planned from the start will be the ending the manga has, but that's a giant fucking lie considering he himself has no idea how long the series will run because *spoiler alert* he has no idea what he's writing anymore, his ending is just a vague goal that constantly changes.
