I use MD as normal, I just ALSO log everything in my own file locally. That way if MD shits the bed I don't lose all my follows. My brain isn't big enough to remember which chapter numbers of 2,500+ mangas I read or not. It would be a pain in the backside to have to go though every manga again checking where I got up to.
I use a program that automatically downloads any new chapters when they're released. Works for most online manga readers, not just aggregates, allowing you to target a scan group's website, in case they do early releases. Pretty damn convenient, honestly.
