Women don't actually have it that rough giving birth. Still sucks that they die from it at times, and in the past it was far more deadly, but it's still not as much of a filter as being male is. Something like 20 - 30% of men end up passing on their genes historically. The rest die before breeding or never get to breed. And if you study history, you'd know why. Shit like the Siege of Alesia, which ended in 1/3rd of all Gaulic men dying ex post facto, and another 1/3rd being enslaved puts a huge dent on the Gaulic population, more so than death via childbirth does.
Also, because even if the mother dies, most mammals (the vast majority of animals that give live birth) have communities that they can be raised in. There are very few solitary mammals.

Concious evolution (intelligent mate selection) have been used by humans instead of natural evolution for tens of thousands of years by this point. It's not necessarily a problem until standards of reproduction are reduced, which they have been. Now you have a majority of women birthing by c-section and feeding babies by formula. Now we're becoming dependent on those things to have kids at all, which is a disaster. If denied access to a hospital, now a woman is likely to die in childbirth because she can't do it without medical intervention.
It's a eugenics disaster and the result of unthinking altruism over the course of generations.
