It's listed on the 201 Form, however, there is a distinction, nothing will technically bar you from a Shotgun License however, certain conditions need to be disclosed and those are listed on the form, just remember that every application is seen on it's own merit however the police's main concern is whether you're likely do harm to yourself or others.
As the 201 Form says itself, if you're at all concerned, please do give your GP a ring in a phone consultation and ask about you potentially applying for a Shotgun Certificate/201 Form and ask them for their advice and see what's on your official record.
An SGC is a shall-issue permit, as long as you fit the criteria put out in the Form they will be forced to give it to you.
