They're volunteers, they know they may not get the actual vaccine, they know the actual vaccine might in fact kill them too, they are ok with that, they are willing to take the risks in exchange for monetary compensation/the possibility of being cured

These placebo/vaccine tests help ensure that the vaccine is 1. effective, and 2. doesn't kill people
