Doom Eternal has a weird ratio on two achievements. One for beating all Slayer Gates and another for unlocking a weapon unlocked by beating all Slayer Gates.
There's like a five percent difference between the two.

Back in the PS3 days, I remember Another Century's Episode R having an achievement that thanked you for installing the game to HDD - which it did in the background while you played the tutorial level. I think it wasn't even optional and technically all took place before you were even allowed to start the game proper. Even then the completion rate for that achievement still sat around ~40% for a long time.

I think if you purchase a game but haven't played it yet you're still counted in the achievement stats.
"Pretty sure"? The term "backlog" exists in common parlance for a reason. Everyone who's had a Steam account long enough to be through one Summer Sale has at least one game they bought but haven't played/installed yet and maybe never will.

I think it's pretty dumb that they measure trophies of people who don't seem to so much as start up the game. I've had GTA3 and VC sitting on my harddrive unplayed for about a year now and I'm contributing to that weird ratio.
Given that there is an entire game mechanic and item dedicated to death, it absolutely isn't a 'journo mode'. It's the intended experience. Just like radiant fights are an extra challenge instead of the intended boss battle, Steel Soul is just an additional difficulty option.
Achievements for Ace Attorney just feels so wrong. Especially for shit like this.
