I would say Ultros' appearance in Final Fantasy 6. In a game that is mostly played straight and serious with mostly tragedy themes, you have this big goofy purple octopus who shows up for a boss battle and squirts ink on you and makes seafood-related jokes and then leaves. He does show up a couple more times in the game but its always very silly compared to what else is going on.

Burt Reynolds in saints row 3. It's so fucking evident they were trying to invoke the at the time nearly decade old Chuck Norris joke but knew they wouldn't be able to get Chuck in. Quite frankly any celebrity appearance where they play themselves and their fame alone is supposed to matter is gay as fuck. What's strange is they made it work in Saints 4 with Rowdy Roddy and Keith David in a shameless reference to They Live, but that's mostly because they gave them something to do.
