I think a lot of people use a mod to make it .22 nowadays. Either Jsawyer Ultimate or just the standalone mod.

I use several mods that make sneaking more fun/immersive. Forget what they're called, but one gets rid of the hidden/detected indicator, one makes NPCs less likely to go "must've been the wind" after they just watched their friend's head explode, and one tweaks your likelihood of being detected based things like how dark it is/if you're in a shadow, how much clutter is in the way, etc; if someone is looking straight at you in broad daylight then they can see you even at Sneak 100.
the ammo in game doesn't really represent anything other than a extra stat, not its actual potential. the varmint rifle, despite being 5.56, is literally fucking garbage after 2 hours or so into the game.

Sounds like enhanced NPC awareness and cover based stealth. Enhanced NPC awareness has issues, though. It'll make some NPCs hostile for no reason. For me it was the Vault 3 prisoners. They'd aggro on me as soon as I attacked one of the Fiends.

it makes sense if you think about it. better to build a town around it, so its protected and control and outsiders can't get to it, rather than leave it in the open for raiders and troublemakers.
