It's aimed at a very specific niche of people. It's not a fun game, but you don't play it for fun. You play it to suffer. Unlike the quintessential "suffering" game Dark Souls, you don't ever get feelings of triumph in Pathologic. Things just keep getting worse and worse. You never feel like a hero or protagonist. The victory only comes at the end and even then, it's not a good feeling.
