It's exactly as Mafia 1. There are no new main missions, but some of them are expanded and some have new side missions included. Now you can visit Salieri's bar and hear thoughts from your gang about your current mission, or objective. This is also how most of these new side missions start. There's like 15 of those new side missions.
Nothing is missing. Even freeride extreme is included.

They still have original Czech voice actors. English one was replaced because they wanted them to do performance capture (which is something most AAA studios do these days) aka it's recorded in the same way as a movie, meaning they do voice acting and motion capture at the same time.
His face was changed because the budget wasn't allowing to use face scan technology that was used for Mafia 3. Those models are all hand crafted. Check out pic related, that's Vito from the end of the game. He looks much more cartoonish when compared to his face in Mafia 3, or even Mafia 2.
Yes it's still quite hard. It's scripted just like in the original game, meaning one AI will always be forcefully pushed to the first place. Mafia's 3 races work in a similar way. Go check them out. I struggled to get 1st places in those.

I'm so hyped but I know I shouldn't be. Mafia was my favourite game when I was a kid, but I haven't played it in probably 10 years, I don't think I'd like it nowadays sadly, since it's (empty) open world and has bulletsponge enemies.
But it's still one of the best video games I've ever played

there's no cockpit camera. Controls are totally different, since they made the game with consoles in mind this time. Inventory is replaced with weapon wheel and there's cover mechanic.
