Being insane is an act. The real Majima is perhaps too much of a nice guy to really make it in the Yakuza world, but neither could he totally abandon that was of life due to it being what he ultimately desired in his heart, and also because of his blood brothers sacrifice. so he adopted a facade to make doing what needed to be done easier and push his morals deep down.
