if the game costs money i think the cheater just loses the cost of it, and would have to repurchase it. Repeat offenders (if found) or official tournament cheaters will both be forever banned.

If the game is free then just go progressive - 2 days, 1 week, 1 month, 6 months, 1 year - permanent. Tournament cheaters also permanent.

VAC is basically just a whitelist, it detects whatever cheats Valve has added to the list. That's why paid stuff like AIMWARE and LMAOBOX don't get detected (unless someone leaks the hack to Valve). Valve also usually won't forget to not ban you for overlays like Discord and TeamSpeak's overlay.
