Konami's big issue was that like a lot of Japanese publishers they just fucking hemorrhaged their big names and didn't really have anybody to pick up the slack.
Capcom can't stick Itsuno on everything so you look in the credits for a lot of their newer titles like RE2/RE3 and MHW and they have a shitload of people in producer/assistant/director roles.
I don't really think there was anything they could've done besides just cut their losses. Was there anybody left at Konami that they could've given the project to to IKEA something into a better game? Probably not. Near the end Kojima was also the one that turned Castlevania into fucking God of War.

Last time I've checked, MGS4, PW, V and even Death Stranding were all pretty successful games. Meanwhile, Fukushima is stuck in Sony Japan Studios doing QA work for shit nobody has ever heard of like The Tomorrow Children, rain, Freedom Wars and the Everybody's Golf reboot.

Kojima may be an autistic little weirdo but the guy can make decent games, on the other hand Konami have cancelled the Silent Hill and MGS franchises and pumped all their money and assets into making pachinko machines. This really tells us all we need to know about their decisions.
Making money requires making good games. That's the problem with a lot of modern indie developers, they don't want to make money so they make mediocre games.

Kojima is a retard but it was Konami's job to manage their own finances. Kojima's always made back the money he spent and far more, they should have either trusted him or reigned him in. Firing someone because you don't want to admit you never gave them a real budget to work with is pathetic.
Nah Kojima had nothing to do with the gameplay on Lords of Shadow. He just gave some advice on the story. Originally Gabriel was way more of an edgelord and Kojima's input made him a heroic figure in a dark world. Without him that game would have turned out like DmC.

And you left out the part where Kojima writes the game plans by himself first and that Fukushima only became involved to help him write dialogue (in fact, MGS1 was mostly done before he was part of the team). The guy was never anything more than a glorified script doctor at best.
