The canon explanation is that the path you take from the boss in the windmill (which is in the direction of the volcano) is a lot longer than the player experiences it as. Iron keep isn't "Above" the windmill, so much as the windmill has a secret tunnel that leads into the volcano/mountain, and the elevator takes you up to the keep.
