Were any nukes used in the Cold War, or any war after WWII? No.
Its an unpleasant fact, but you can only avoid use of nuclear weapons by having 0 countries with them, or 2+ countries with them. At this stage, you can not guarantee getting everyone to give up their nukes simultaneously and honestly. Disarming risks putting the world in a situation where only 1 side has nukes, and no longer any equal and immediate deterrent against using them.
