thing is it is cannon in fallout 2, you can find some entries about some of the vault and the mishap.
not to mention the waterchip breaking may have been intentional.

A vault where cross breeding between animals and humans is required for all members. Except the only animal in the vault is a horse with a 3 foot cock.

I would make a vault where all the children are indoctrinated and exposed to nothing but manga and anime. From early age on everything they see, including school material, will be through the lense of anime. The Vault would feature combat training in the style of DBZ (obviously without the super powers, just the mannerisms) and Naruto (use of katanas and shuriken etc). This would go on at least 4 generations, so that there's no way that the original generation of adults supervising the children could taint the younger generations with their old-world western values and rationalities. After 4 said generations, the Vault would open and the inhabitants would be released into the wasteland.

So what's the experiment? To see how well they would fare in the outside world while acting like retards? I mean Fallout 4 had a similar idea except they were trying to breed super efficient warriors/merceneries from childhood it didn't end well for the Vault.

The revolt and escape was part of the plan, or at least a forseen incident by Vault-Tec.

There's two vaults, both connected, but unable to even comminicate with each other.
