Hitboxes use simple shapes because they are quick for calculations. A sphere needs to be a single point in 3D space and a radius, that's it. An entire polygonal mesh for a character needs to be about 10,000 points in space. When you have to calculate this every physics step it becomes very resource intensive.

Such an approach may only be feasible for simulations, but for real time games, you simply don't have enough resource budget to waste on "perfect" collisions with the current methods, algorithms and approaches.
Collision detection is one of the biggest problems in computer science, because there's no perfect "one size fits all" algorithm yet, and the ones that come close, are too resource heavy. Every single approach has its own benefits and disadvantages, making them fit for specific tasks only. Every year, these are still being researched and developers, computer scientists and mathematicians are still looking for better alternatives, since collision detection often plays a big role in complex computer simulations and a lesser role in game development.
The thing is, many new improvements and methods surge every year, but nothing groundbreaking. Most of these improvements are barely noticeable to end users.
Games still use hitboxes composed of simple shapes, because it is "good enough" and it is not resource heavy. Once there's a new groundbreaking collision detection model with better efficiency, we shall see it being implemented in newer games which could benefit from it.
