Kingdoms exist everywhere. Lordran was the start "first" on the surface.
The weird thing about the DaS intro is that Hollows found souls in the Arch Trees, not only that, but built weaponry and armor within the tree to take the surface.
My whole headcanon is that people ascended to dragons to outlast the age of fire and dark, they turn to survive. They dominated the surface to keep men docile. If someone wandered to the surface they were dealt with.
The intro is ash, Das3 ends with the world in ash but not a tree in sight.
Men, gods, survived the age of ancients as hollows in the trees, and without trees to hide, life will not survive.

Demon Souls is totally and completely unrelated to the rest of the games just as the other titles mentioned are.
I didn't understand what was happening, so once I beat Gwyn, I did what I had been doing for the entire game: rest at a bonfire.

dark souls takes place in a greek/roman style underworld. In the age of ancients the world is flat, grey, empty and timeless. People (hollows) exist, but at most they shamble around, perhaps talk to each other, have names, but there is no structure, no civilization, no time, no memory. This is very similar to the greek underworld, where the dead simply exist forever in endless wastes. With the arrival of fire, they're able to gain power and create civilization, but as the fire fades, the world reverts back to its original state, grey and timeless, humans exist but have no distinguishing features or culture. The entirety of the game actually takes place in the underworld of a true world, which is demon's souls. As the age of ancients is like a greek/roman style underworld, demon's souls feels closer to a greek/roman style universe. The architecture, armor, weapons, etc. all feel more classical than medieval, bosses like the phalanx support this. Dark souls takes place at the end of the demon's souls world, where all humans are now dead and in the underworld.
This explains some of the unexplained questions from the opening cinematic of dark souls, like where the hollows initially came from, and why the lords have names. Gwyn, izalith, etc. all had names in life, and perhaps remembered them in death, and kept their same names once they got the lord souls. It also explains how Nito can be the first of the dead, in a world without death. He was actually the first person to die in boletaria, and the first person to end up in the underworld, giving him the previously unexplainable title, first of the dead.
