Environmental cues, like certain music start playing when said unkillable monster starts hunting you down
Monster design should invoke primal fears, like the monster has huge teeth or claws
They shouldn't instakill you, but they shouldn't let you walk off scot-free either

It makes sense, the premises of horror games just dont go the extra mile to make something interesting that would fuck with the players mind. Its always the same thing, ghosts, monsters, zombies etc. but not much else to it. There is so much more that could be done, something really fucked up and interesting. I just like a good premise, like Soma or Silent Hill 2.

Overpowered and fast enemies, sound design that makes enemies and ambient sound similar to make the player fall into a state of paranoia, safe zones that randomly disappear when you try to run to them
horror games don't faze me in the slightest - Alien Isolation, RE7, Silent Hill 2 - None of them really get me spooked despite my best attempts, like playing at night with the lights off and headphones on

For me sound design is the biggest thing that makes a game scary, the visuals and setting definitely help too. Gameplay doesn't factor into it at all for me since dying in a game means nothing but losing a little time.
