It is just that difficult. I can remember doing so years ago on an actual DS.
Make sure your emu is accurate first of all, there's no glitch or anything. Watch a playthrough on youtube to confirm you're not dealing with some glitch and give you ideas for tactics. Then, if you're not doing it well with the mouse (which should be okay) consider using a emulator that works on a phone or tablet of yours. For instance the awesome open source Citra now works on Android, a well as desktop PC. Not sure about plain DS emus. DeSmUmE?

Spam gel frequently. Just select the gel, cover the screen with it real quick, and then do whatever you need to do for like 2-3 seconds and repeat. Kyriaki is probably the hardest, desu.

Went and watched a YouTube vid, turns out you have to be real quick and spam the gel. No issues on my emulator, I'm on drastic DS and everything is fine. Since it's on a Samsung Note 9 I even have a stylus replacement, which is great

I'll also never get why they crap with the names on translation. "Trauma Center" sounds too realistic/serious etc...for a plot about magical Hippocratic abilities, bio and nano-tech customized infections etc.. and more. The original "Super Surgical Operation Caduceus" is WAY better.
Also, I have to wonder is there a fan translated or undubbed version of the Wii games?
