When Geno took off as a Smash request SMRPG, specifically forest maze, was becoming a viral meme online and nostalgia for the game was pretty strong. The fact things like the niconico videos, super mario bros z and the newgrounds amv coincided with the pre-release hype for Brawl was a stroke of luck that worked in his favor. He was the standout character of a game that was in the public conscious again on message boards (not most important, just the most memorable and popular of the games OCs), and had a moveset that really naturally lended itself to Smash.
He might have died off the same way other dated Brawl requests like Vaati did but Sakurai poured fuel on the fire by saying he considered the idea of adding Geno, even if briefly, so the hope for his cult fanbase never died off. Plus there's the taboo/exotic element of being owned by Square that makes him more exciting. Since then it's just snowballed, wanting Geno is now just Smash fandom culture.

The ironic thing is that if Genofags ever got their wish, it would ultimately be a death sentence for their meme puppet. The bandwagon is the only thing keeping him relevant. So his main unique mechanic and gimmick would be timed hits adding/changing properties of some attacks if the right input at the right time of an attack animation.
I'm guessing they'll pepper in other unique quirks like if Geno Boost is the Up+B it'd have a buffing property, a low profile taunt useful for dodging projectiles etc.

Well we do have a precedent, when Sakurai wanted to negotiate for Cloud the dumb fucks tried to push Lightening instead, but Sakurai played hardball and insisted on Cloud. I believe if Geno did well on the ballot (he did) Sakurai would have approached Square, and I further believe Square would have pushed Lara Croft, 2-B, or even Sephiroth over catering to a fanbase they do their best to forget exist.
