My headcannon is that Skullface was never in the MGS3 location until something like a day after Snake had left. It's really undermining to certain parts of the plot if you are supposed to believe Skullface was there the whole time.

That seemed more like stylistic slow mo rather than Quiet actually taking a long time to aim a gun. Kojima uses the same sort of zoom-ins and slow no all throughout the game. This could even be explained in universe as Venom's adrenaline kicking in and everything moving at a glacial pace as a result.

Why? There's other nurses everywhere in the hospital cutscenes. It makes perfect sense for Quiet to infiltrate the hospital that way. It makes perfect sense for them to sent in a woman, too. Putting her in a soldier outfit and giving her nothing but a garrote wire to go with all her tactical webbing is much more stupid than giving her a nurse outfit.

It was also a private hospital and the staff would have certainly noticed a stranger wearing a stolen uniform. Also she had other weapons than the garrote.

That final flashback where Venom is literally unconscious while Ocelot pulls the real Big Boss out of the car in conjunction with the final two tapes where Ocelot straight up tells Miller that Venom isn't the real boss and the other one where Venom plays the track and Big Boss tells him to build up the Diamond Dogs basically proves that Ishmael was real.
