1. It was conditionally certified - allowed to be printed and shipped on the condition that a day-1 patch would pass certification. But their current day 1 patch still doesn't pass
2. It was certified but Google gave them a fat wad of cash and said port it to Stadia at launch

Incredibly likely. Delays after the game is boxed and ready to ship means there's some catastrophic shit going on, like the game's bricking consoles and the patch doesn't fix it. It means that the game is in it's "final" state and is shipped off to the factory that prints disks. Typically after it's gone gold game dev studios keep working on their day 0 patch to fix any non-game breaking bugs that may remain.
