Here is what some people have been saying. Basically there was some issue with certain PS4 models, and the bugs are on the discs they've already pressed. The issue was already fixed and it can be rolled out with a patch, but it still caused a delay. Of course there is no proof for these, but it sounds a lot like what CDPR said as well, just in a more avoidant way

I can't imagine what difference 21 days could make when the game is complete enough to print onto discs, but is apparently so unfinished that it needs to be delayed.

The only explanation is that the team is extremely incompetent and don't seem to care much about the overall game, just their department of development.

If it's an issue that can brick a console, you can't ship it in that state on a physical copy. It's possible that the guy who got the disc doesn't download the patch, and then his console is dead
