When the hunt ends and Gehrman frees a hunter from the Dream, the Moon Presence descends from the moon to consume the victor, draining them of their blood. This is why another name for the Moon Presence is "Paleblood", which has the meaning of "a corpse drained its blood." In essence, the Moon Presence is similar to the Old One from Demon's Souls, insomuch as it's a big hungry mouth that's delivered souls (blood) by its archdemons (hunters), which thereby makes it a vessel for immense power.
