Maybe have the option to kill or spare abby and if you kill her then Ellie loses her fingers, her family, and will likely have Lev hunting her down next. and if you spare her then Ellie still has lost everything up until this point but hasn't lost herself and can grow.

I see what you're trying to say, that Ellie grew just like joel did, though you're being intellectually dishonest about. Joel was never under the impression that they were going to kill Ellie. So that discovery combined with his growth leads to him organically making the decision to save Ellie. Ellie on the other hand was so hellbent that she lost her family and mere moments earlier she was holding a blade to a child's neck so she could defeat abby. So it comes off as inorganic for Elly to go "You know what, this isn't working for me" while pumped with adrenaline and holding abby's face underwater

Imagine if Joel severed ties with his brother and took a hardline stance that he will kill anyone that gets in the way of him getting that vaccine. He's so enthusiastic about the vaccine that he even asks to sit in on the surgery. and it isn't until the incisions are being made that he suddenly decides to abandon his hardline pro-vaccine stance and kill everyone to save ellie.
